SPOT-THE-SCAM STORIES DOCUMENTARY
Human Reclassified Foundation – Contributor Video Release & Consent

Thank you for sharing your story! To protect both you and Human Reclassified Foundation (“HRF”), we need your permission to use the video (and any related materials) you submit.

1. GRANT OF RIGHTS
   I (“Contributor”) grant HRF a perpetual, worldwide, royalty-free, irrevocable license to:

• Use, reproduce, edit, adapt, publicly perform, display, distribute, transmit, and create derivative works from the video, audio, photographs, transcripts, or other content I submit (“Material”).
• Combine the Material with other content and use my name, likeness, voice, and bio in connection with the Material and resulting documentary, trailers, educational resources, social-media posts, fundraising campaigns, grant reports, and other lawful nonprofit purposes.
  I waive any right to inspect or approve the finished works.

1. NO COMPENSATION & NO OBLIGATION
   I understand I will receive no monetary compensation and HRF is not obligated to use the Material.
2. REPRESENTATIONS & WARRANTIES

• I created or own the Material and have authority to grant these rights.
• The Material does not infringe any third-party rights and is not defamatory.
• Anyone recognizable in the Material has consented to be filmed, or I have blurred their likeness.

1. DATA & PRIVACY
   HRF will store my contact info securely for project administration and impact updates. I may request deletion of my personal data (not the published Material) at any time by emailing privacy@humanreclassified.org.
2. PARENT / GUARDIAN CONSENT (if Contributor is under 18)
   Parent/Guardian Name, Signature, and relationship are required if Contributor is a minor.
3. GOVERNING LAW
   This agreement shall be governed by the laws of the State of California, USA.
4. SIGNATURE
   By checking the e-signature box and typing my full name in the form, I confirm that I have read, understood, and agree to all terms above.
